Who the Breg Polar Care Wave is for
Pick the Polar Care Wave when your surgeon wants you on cold and compression at home: after a total knee replacement, ACL reconstruction, hip arthroscopy, rotator cuff repair, spine surgery or an ankle fracture repair, for example. Breg states the Wave is intended to treat post-surgical and acute injuries to reduce edema, swelling and pain where cold and compression are indicated. If your protocol only calls for cold, the Polar Care Cube or Polar Care Kodiak is the simpler choice.
What comes in the box
- Wave cooler: container plus the lid that houses the pumps and control buttons
- Tubing with snap-lock connectors
- Wall power supply
- One Breg cold compression pad, the one you choose above (knee, large knee, shoulder, hip, back, ankle or universal). "Cooler Only" ships with no pad.
Sold separately: Wave ice bags, the Wave Y-connector for treating both knees, spare Wave pads and a replacement power cord. Cube, Kodiak and IceMan pads do not connect to the Wave.
How it works
Pour cold water to the water line, add cubed ice up to the ice line, latch the lid and plug the unit in. One circuit pumps ice water through the pad. A second circuit inflates the same pad and lets it relax again, with each squeeze-and-release cycle lasting roughly one to two minutes. Cold and compression each have their own button, so you can run either one alone or both together, whichever your prescription says.

- Cold: Colder (pad averages 45°F or above), Cold (pad averages 50°F or above) or Off
- Ice: Breg says to top up ice every 6 to 8 hours or as needed. With the ice bag accessory, swap in four frozen bags every 4 to 5 hours.
- Auto-off: if the power is on but both therapies have been switched off for over 15 minutes, the unit powers down
- Clearance: FDA Class II 510(k)

Two things owners mention
No shut-off timer. The Wave keeps running until you turn it off. Its only automatic shut-off is the 15-minute rule above, which applies only when cold and compression are both switched off. Some owners also report that a smart plug or outlet timer won't restart it after the power cuts. Unless your prescription covers overnight use, plan sessions for when you'll be awake to stop them, and set a phone reminder for skin checks; Breg says these are typically every 1 to 2 hours.
Ice bags can tear. Reviewers say the reusable bags can split with repeated use. Breg's instructions are to fill each bag only to its water line (about two cups), never overfill, and dry the outside before freezing. Keeping a spare set in the freezer helps, and plain cubed ice still works if a bag fails.
Buying vs. renting
Rental firms usually charge cold-compression units by the week and take them back afterward. The Wave is a single-patient unit you keep. Is the Wave worth buying instead of renting a Game Ready?
Both pair cold with intermittent compression. The difference is ownership: the Wave is sold to you outright and stays with you, while Game Ready systems are commonly rented through clinics. Your surgeon may have a preference, so ask before you choose.
Do I actually need compression?
That's your surgeon's call. If you're told to skip it, set compression to Off and run cold only.
Can I use frozen water bottles instead of ice?
Breg's instructions say to use cubed ice and water, or four Breg ice bags plus water, and nothing else.
Wave or Cube: what's the difference?
The Cube is cold-only, runs about 6 to 8 hours per ice fill per Breg, and uses WrapOn pads. The Wave adds adjustable compression and uses its own cold compression pads. Pads don't swap between the two.
Prescription, warranty and returns
Under U.S. federal law the Polar Care Wave may be sold only by or on the order of a medical professional, so follow the frequency and duration on your prescription.
